copy of nic for visa app. from pakistan

who can translate and sign the copy of national identity card (NIC) from pakistan?
on which type of paper it should be translated?
Can I translate using a normal paper?

Notarized Paper. I did that for my CNIC. From Karachi I did this at Civic Centre. -

just visit civic centre or city court.. you find many of them sitting outside...

They have a format on a paper of NIC ... birth certificate etc..

Its not a simple paper it is pre formatted they will just fill it
