Confusion in submission to visa office

Can someone please answer these confusing question:
Q1. I have a salaray certificate that only says monthly salary but I have nothing about yearly salary. As annuual salary is monthly salary *12. Is this a valid proof
Q2. I have already send the following to CIC.
APPLICATION FOR PERMANENT RESIDENCE IN CANADA (IMM 0008)
Check that it is completed by the principal applicant.

SCHEDULE 1: BACKGROUND DECLARATION
SCHEDULE 3: ECONOMIC CLASSES – FEDERAL SKILLED WORKERS
DO I need to refill these forms and send them to visa office.

Q3: Which documents to be notarized and which documents to be sent in Original.
Q4: What are the forms that are required to notify the chage of email address.
Q5: Is it necessary to send birth certificate.

Please someone answers these questions:

1) This should be okay but does it mention somewhere that is a monthly salary. Perhaps you should write it in your cover letter. Did they ask for salary certificate specifically. You could also show your tax document which should state your yearly salary.
2) I think most people probably re-fill it, correct any mistake and date it to the current date.
3) From my memory, the originals should be PCC, work experience letter, IELTS, transcripts but you should check appendix A for your visa office to make sure.
4) I don't think there is a specific form. The best is to update it in the form AND make sure you write that in the cover letter to point it out. If there is a specific form, someone else could hopefully reply to your thread.
5) Yes, I believe so. If you do not have a birth certificate, you could use other documents but I believe this could be nationality specific.

Actually I dont have reference letter, I only have experience letter that mention my duties. I have a separate salary certificate. Is it necessary now to obtain refernce letter.

i meant experience letter. I asked them to mention the salary on the experience letter but if yours is not mentioned then salary / tax documents should be okay.

From CIC website they have written that we should submit the photocopies of forms that we have send to CIO. But you have said that most people refill it. Its confusing me. Is it a optionl to refill or send them as a photocopy.

Also If I send my original transcripts when I will get them back.? Have you submit your original transcripts.

i refilled the form correcting some mistakes and i sent in a photocopy and marked which was the photocopy of what i sent to CIO in NS.

I haven't gotten a visa, so not sure what i did is correct or not. Yes i did submit my original transcript but since i have other originals, from the universities, it is okay if they do not return it.

Try to start your own topic. That way we won't be discussing two different things in one thread.

1) application forms
2) check
3) address labels
4) copy of the biopage of the main applicant's passport.

But check www.cic.gc.ca for more information because rules may have changed since i applied.
