confused with additional dependents form

just completeing all my paperwork and a bit confused

[size=10pt]Guide says that this form is to be completed by the principal applicant on behalf of each of the dependants
Checklist says - to be completed if applicant has more than five depandants[/size]
which is correct????

if your kids are more than five then you have to fill this form on the behalf of each dependent other vise the generic form is sufficent.
