Conflicting POLO requirements

Hi everyone, as an employer, I am just beginning to get familiar with the requirements for POLO. My caregiver Philippines has been issued a VISA and now I am about to go to POLO to get my documents verified.

Based on requirements posted on their website I need the following
philcongen-toronto.com/consular/docs/POLO2014B.pdf

However, I also found the following requirements
philcongen-toronto.com/consular/docs/POLOCHECKLIST.pdf

My question is about item number 8 and 9 of the second link. It mentions the following requirements
8. Proof of purchase for the airfare as paid for by the employer (include copy of the
invoice and itinerary)
9.Proof of purchase for the gap medical health coverage for 3 months of at least
CAD$50,000.00 coverage (include confirmation of coverage)

Are these still necessary requirements to go to POLO. It looks like the one they replaced 8 and 9 with an "Addendum to the employment contract" form. (of course, I will still purchase the ticket and buy health coverage, but at this stage of process, are these required?)

can anyone recently going through this verify? I would like to submit my documents, but wouldn't want to have an incomplete package.
