Concern with Other Name.. Help!!!

While filling up the form for my wife, I have to mention her name before marriage.. Now, last name seems to be a mandatory field here however; my wife did not use her last name in her official documents (due to a miss during her school days)..

What should I write in the Last Name field??

Mention her last name which is the recent last name in use i.e. post marriage one in the EE profile.
If in India get a letter drafted from a lawyer or at a notary that her nee name used before marriage and in school and her name in use now refer to the same person. (You will get the templates of that letter in this forum) and have it notarized.

While filling the Express Entry form mention that she has been known in more than 1 name as yes and enter the names in the appropriate field.

I don't think I can do that because in this case current name and last used name would be same.. This would seem like why did I chose that she had another name before marriage.. And that can lead to misleading information..
