Completing Stat Declaration of Common-Law from Brazil

Has anyone ever tried to do the stat declaration of common-law form from Brazil. Can we do this at the Cartorio even though the forms in English? Do we have to then officially translate the notary stamps into English?

We were hoping we wouldn't have to do this but we just got a request from SP visa office
