Common-Law Application and OWP Questions. Please help!

Hello everyone!

I am currently filling out my PR (Common-law Sponsorship) and OWP application and I have run into some bumps that I'm hoping someone can help me with. I apologize beforehand if my post gets a little long. First time posting so if I make any mistakes in the use of this forum please feel free to let me know and I'll make the changes necessary.

-What constitutes a statutory declaration and are they necessary?
-Mindset towards your relationship? What are we supposed to write down on this, I don't seem to find a good explanation on this, if anyone could share a template or example with me I will greatly appreciated.
-Under Family Information it asks for names and address of my parents. Due to personal reasons I do not have any information of my Father, I usually fill out all the information for my Mom and leave my Father's blank, would this be a problem? Or what would you recommend for me to do.
-SO and I are engaged, we didn't have a formal engagement ceremony other than the proposal, does that count as a ceremony?

For the OWP I was told I'm supposed to apply for the Application to Change Conditions (IMM5710)

-In the Document Checklist asks to specify whether you're currently a Student, Provincial Nominee, Refugee, etc... I am currently just on a Visitor Visa, which is not listed on the form. Are all of those sections to be left blank?
-At the top of the IMM5710 Application, 3. I am applying for the following... Which would apply to me? I had a Post-Graduation OWP that expired last year.
-Document No. On most recent Visitor Record. My last document is my change of status of worker to visitor, it has a big red number at the top right corner, another number on black print right under it and an application number. I'm not sure which one is the right one.
-Details of Intended Work in Canada. I obviously do not have a job lined up since this OWP application is being done with my PR application, should this section just be left blank?

Lastly, if you could give me an example of how to file these forms and send them it would be great, I'll probably try to organize them in a binder and divide them in sections with the documents for each form attached. If anyone has some pictures of how you sent out your application it would be great help.

Sorry again for the amount of questions, I am doing the application on my own and the forms are overall easy to follow, I just found myself a little bit stuck in these sections. Thanks to whoever can give me a hand!!
