Hello,

The doctor gave Information Sheet post doing the medicals. On the Sheet the client Identity Details he used my Michigan Drivers' License and not India's Passport. In my eAPR online application I am not providing my Michigan License anywhere would that be problem. Or that information should contain India's Passport Information.
