Citizenship: can anyone share medical waiver form?

Hi,

I have a family member who applied for citizenship in 2019. This family member is severely autistic. I believe IRCC sends a medical form to be completed by a physician. Can anyone share this form with me? I am waiting for them to send me this form but I am trying to get ahead of this and get it completed by a physician.

Hi did you get you process done?
My autistic son could not do the oath on the day and they said they will send a medical waiver form and untill now I have not had anything.

Did they contact you?
