I know I can apply for citizenship by descent since my mother was born in Canada. But, I have a question on the application - Question 3 B.....it is asking if I would like to request a different name (other than my birth certificate). As a married woman, I don't know if I should put my current last name or the one on my birth certificate.
I have both documents to show the change of last name, but, I don't want to make a mistake. Thank you.
 
At the top you put the the last name that was given to you on your birth certificate. If you want your married name on your canadian citizenship certificate, then you will need to provide a copy of your marriage license and birth certificate.
