Citizenship application - personal identification documents

Hi,

I understand that PR can be used as a personal identification document only if the other personal identification document is also issued by government of Canada. This means that Indian passport & PR card CANNOT be used as the personal identification documents .

can someone confirm if the following combinations can be used as personal identification documents

1. PR card and OHIP card
2. Indian passport and OHIP card

Option 2. You can also attach the driving license of Ontario

The Passport and PR card have my spouse's maiden name, other Canadian documents ( DL, OHIP card) have assumed husband name as last name.
Is it Ok or do we add a copy of marriage certificate to show connection between maiden name and husband name?
