CIT002 (10/2017) Dates problem

Hello,
1. While filling this CIT 002(10/2017) form in section 10 a and 11 while putting dates just year and month appears on print out.
If one uses the button (down arrow) next to the field you can insert date 10/12/2014 but printing it only 2014/12 appears.
Is this O.K?. Could someone please clear this issue.
2. Also in taxation question 12b. Required to file & taxes filed . Do I need to put YES to both. I have filed from 2012 to 2016. So taxes filed will be YES. How about required to file?

Hi,
1) Only the month and year are needed for those 2
2) You can fill your taxes even if you're not required to. Since you did for the 5 years, just don't bother and put yes everywhere !
