Cit 0002 question 3(a) Canadian citizenship application

3(a) if you applied for Canadian citizenship before. If you submitted an application for citizenship that was returned to you as incomplete, answer “no” to this question. Iam confused with this question as i applying for first time for citizenship should i answer no or yes. For section 5 I added surname to my name before coming to canada and need to give both the names with surname and without surname. what option i choose for surname added.name at birth or ?Thanks in advance
