Hi,
Yesterday my spouse received an email from CIC asking to provide an original police clearance certificate from Singapore. The email comes with the following two attachments:
1) All - Singapore - 1 of 2 - Police certificate request - EN (word file)
2) All - Singapore - 2 of 2 - Application for appeal - EN (pdf)
In the email CIC advised that obtaining Singapore police certificate is a two-step process.
Step 1: Email Police certificate request - EN (word file) and Application for appeal - EN (pdf) form along with a copy of passport to Singapore Police Force
(Once Singapore Police informs applicant of the result of Application to appeal, then proceed to Step 2)
Step 2: Submit to Singapore Police Force a full application for a Certificate of Clearance and the requested documents.
Apparently, judging from the body of this word doc Police certificate request - EN (word file) it seems that it's supposed to have the applicant information filled in by CIC, so that it could serve as a proof of request from CIC to Singapore Police Force suggesting that CIC needs the police certificate for a given applicant, as part of the immigration application process.
However, the word file is missing any applicant identification info; such as applicant's name, DoB, UIC#, Application# etc. It has headers for all of these fields but no data is input there. Please see below the word file as received in the email.

The Singapore police are no longer routinely issuing Certificates of Clearance to non-citizens. In order to obtain a Certificate of Clearance you must submit the following documents to the Singapore Police Force (SPF):
• A copy of this letter
• The appeal form attached to this e-mail (PDF document)
• Copies of your current and old passports or other documents confirming your stay in Singapore

It can also be submitted by postal mail to:
The Singapore Police will advise you of the results of your appeal within 15 days. If it is accepted you will have to forward to the SPF a full application for a Certificate of Clearance and the requested documents. Please advise our office by e-mail of the results of your appeal as well as the date your full application for a Certificate of Clearance is submitted.
Once you have notified us that your full application has been submitted to the Singapore Police Force, we will follow-up with the SPF regarding the final results, which will be forwarded directly to our office.

Now, I am wrestling with the following thoughts,
a) Should we contact CIC to find out if they missed filling out the applicant's details in the word doc, and ask for a replacement?
or,
b) Should we attach the word doc as it is (with blank fields) along with a copy of CIC's email (the body of email has applicant's details and a line suggesting CIC needs Singapore Police certificate), in our email to Singapore Police Force when submitting Application for appeal ?
or,
c) Should we fill out this word doc ourselves, before sending it over to Singapore Police Force ?
It's much appreciated if someone on this forum who has encountered a similar situation can share his experience or anyone else who can provide some guidance.
