Child born abroad proof of citizenship and limited validity passport

Hi! URGENTLY need someone’s help!! My baby is born abroad and I am Canadian. I am applying for her proof of citizenship and I need the limited validity passport at the same time as I need to travel soon. Do I submit pptc042 as well?? i am confused because in section 4 it says proof of birth certificate or citizenship… but I am just applying for her so how can I fill that out or do I just need to submit pptc 445 and 116 along with cit000 form? Also is a copy of my birth certificate okay? Unfortunately my original is laminated I didn’t know it would be invalid if it’s laminated so I can send a copy instead
