I know with my application ( applying out of Mexico City) there is 1 on the Canadian immigration website, and then one on the VAC website. Only one is used with immigration, but if you are applying through a visa application center, there may be an additional one for them. (in my case, it was 3 pages) not sure if this is what you are referring to...

Goodluck!

I would submit both checklists. On the second link you gave it says this:
"Many visa offices have specific local instructions about which documents you must submit with your application. Consult the list of Canadian visa offices to find the office responsible for your country and what requirements and procedures you must follow when you apply."

you went directly to the NYC site, and they listed that as a requirement - so definitely send it. Either way, it wont hurt your application if the end up preferring one over the other.

In regards to how long it will take to get your passport back, in looking at the NYC site, even if you take it in person it could take approx 30 days. It also says to apply 3 months in advance due to a new system they are putting in place or somehing.
The info is here:

Well depends really on what visa office you're applying from. If you're applying from the US then the first one holds. Different visa offices have different requirements. check the site of the office responsible for your region. And send all that applies to you.

The problem is as usual, there is no clarity.

It is my opinion that as both sites (CIC and country site) refer you to each other, send both forms as suggested by itscoezy and job_seeker.
Or you can call the office that you are applying to and ask a direct question.

Im not sure how the below does not answer your question about the second checklist you must fill out... it says this DIRECTLY on the link you gave.... you MUST fill it out. You already know you have to submit all the documents on the canadian immigration website. And when you read the below, you now know you have to complete all the documents on the visa offices website, so I'm confused as to why you are still wondering :S.

"Many visa offices have specific local instructions about which documents you must submit with your application. Consult the list of Canadian visa offices to find the office responsible for your country and what requirements and procedures you must follow when you apply."

I think you didn't even bothered to open the checklists from the sites you sent. Look closely that the documents asked for in check list from cic.gc.ca website is also asked for IN THE checklist from the US visa office. In effect the check list from NY is a super set of the checklist from the cic.gc.ca. Print both forms and look at them closely.
