My friend got accepted into a Canadian college and started in Winter.
3 months into the course and she want to shift to other program.
She want to shift from cooking to the medical field.
To finish the 2 year course she will go beyond the expiry date of the student permit?
What is the chance she will be refuse because she shifted from one career to another?

It depend when second program starts. It should be fine if waiting period is less.

She will end the term this april then the other course will start at Fall 2017
