Can't fill out IMM5713E

Good day to everyone on this forum, I've been trying to fill out this IMM 5713E form for a family representative but it won't work on my computer. I've tried it on Acrobat and it says "You cannot save typed data in this form". So I assume we print it out and fill it out by hand? I have indicated on the questionnaire that we are traveling together as a family with my husband.
