Hi, I am a naturalized Canadian citizen living in US. I just had a baby in Jan 2017 and would like to apply for his Canadian Citizenship certification, but have some questions on how to fill in the forms:

1. CIT 0001 Section 12 - Appointment of a Representative - Since I am technically applying for my son, do I need to check "Yes" for the question "Are you appointing an individual, firm or organization as your representative?"?
2. Do I need to fill out a "Use of a Representative" form (IMM 5476) since I am applying on behalf of my son? If so, what to do in the Signature section?
3. Since the baby cannot sign the CIT 0001 application form, I assume we leave the "Signature of applicant" empty and I will sign the "Signature of parent/guardian" box, correct?
4. They asks for 2 ID's (1 photo) for my son. I plan to use the US passport for the photo ID, but not sure what I can use for the 2nd ID for a newborn. Any suggestions?

Note that any original documents you send with your application will not be returned. If you live in a state that allows notaries to certify true copies, you may send those instead. Look for your state here:

They can also be certified at a Canadian consulate.

Another question, it says in the document checklist that for Scenario 3 (If born outside Canada on or after Jan 1, 1947 to a Canadian parent) that we should send in the Original copy of all citizenship/naturalization certificate(s) if applicable. Does this mean sending in my (mom) original Citizenship card? Or does it not apply since the baby doesn't have a Citizenship cert/card?

That section refers to the child's citizenship certificate. In this case there is none.

However, you do have to prove that you were a Canadian citizen when the child was born, which would be your birth certificate (if born in Canada) or your citizenship certificate/card. They want a certified copy, because originals are not returned.
