i just applied for express entry. Now i am planing to go back home for 5 weeks. Now my question is that when i come back to Canada, do i need to submit any document or any PCC to CIC website.
can we travel to home country while express entry profile is in process

There is no travel restriction for travel from one country to another. What comes to my mind is, if you are traveling to India and will be staying there for 2-3 months, you must update CIC about the change in the situation (address change). You can do this by raising a case specific enquiry, the link is available on the CIC portal.
My question is that.. its complusory to upload the documents or i can exempt from this.

You only have to notify IRCC if:
-You changed contact info
-Changed Address
-Changed Country of Residence
-Change in family composition
etc...
