Can we give medical tests in two different countries?

If we have medical exam in one country, can we give re-medical in another country? Obviously the physician would be different in this case.

Also, can we request for a delay in medical exam or is it not advisable? (I mean beyond the given 45 days)

Some one with personal experience or who knows someone who did this, please advice.
