Can I do medical exam when outside of the province that nominated me for PR?

I am nominated for PR by Ontario and have been asked by IRCC to do the medical exam in the next 30 days. Although my permanent address is in Ontario, and I decide to keep living there, right now, I temporarily stay in Alberta where my wife studies. My question is if it is OK to take the medical exam when I am outside Ontario or if they (IRCC) take it as a sign that I have left the province and refuse my application.

Yes it's fine, they cannot refuse your application based on assumptions. People do medicals out-of-province all the time because, for example, they could be on holiday in a different province or country when receiving the medical request.

IRCC has a high bar to meet if they suspect you're not living in the province of nomination, and in order to meet that bar they would be asking for additional information such as bank statements/lease etc. to verify your permanent address. After AOR, IRCC aren't allowed to refuse your application without asking for additional information.
