Bringing to Canada more than 10K CAD

Hi All,
Would you tell me where I can find a declaration form that should be filled on the border if I bring to Canada more than 10K CAD?
If any of you has this form or know the link, where I can find it - please share.

You will sign this form at the border/port of entry at secondary inspection, in the presence of a CBSA officer. The officer will retrieve and fill out the form for you to sign.

Just answer that you are carrying over $10K and the officers will tell you where to go.
