BOWP Documents - CEC Applicant

Hey Guys,

Anyone over here, who got their BOWP approved from CIC?

What documents did you guys submitted with your online BOWP application? So far, I have gathered the following:

1 - Application to Change Conditions, Extend my Stay or Remain in Canada as a Worker (IMM5710)
2 - Passport
3 - Digital Photo
3 - Letter of Explanation and EE-AOR

Also, one of the questions in the Come to Canada tool is, Medical exam performed by an IRCC doctor within the last 12 months? Should I be answering Yes or No?
