I have an arabic birth certificate from Qatar which is translated by a certified translator in Canada.
I just found out after translation, that my current lastname on my passport or previous passport is different from the lastname in the birth certificate.

In our country your lastname is always your grandpa'-s first name, so your father and you will have a different last name

For example if the child is Called Jacob, and the fathers name is Jack Adam Johnson, then the full name of the kid will be

Jacob Jack Adam. In this case the last name will be Adam instead of having some lastname as the father Johnson

The Qatar Authorities when they were issuing the birth certificate they didn't realise our tradition of naming and they gave me th lastname of my father.

Then i never used that last name, and i used my grandpa's nickname as my lastname for almost all my life until 2 years back i decided to correct my lastname with the one i was supposed to use which is the name of my grandpa.


Now my documents seems a bit of a mess, a birth certificate lastname is different and then i used some kind of nickname in my lastname on all my passport and other official documents ,and then now my passports or any other document after 2012 have my correct lastname.


With the change of name thing, I was ok with that but now a third thing of mismatch may create confusion and suspicion to the Visa officer.

What do you guys thing? i read from this forum that the main points of the Birth certificate is the date of birth and parents name and first name is important, while lastname are not very important.


Beside there issuing authorities are unable to issue a replacement because of this name corrections. and i don'-t have any other proof other than my passport which i read from this forum that it can not replace the Birth certificate.
 
So your birth certificate has a different last name than the other documents right?

What I would do is first of all, on form IMM0008, under Personal details, question number 2 asks you: "Have you ever used any other name? (eg nickname, maiden name, alias, etc)
Tick that as yes and put your name from your birth certificate, then make an addendum to this question, on a separate piece of paper and explain the situation just like you did here.
