Hello everyone,

I have the following documents:

1. I have a copy of my birth certificate in English (black and white, I lost the original).
2. Affidavit from my parents, stating the following:

To whomsoever it may concern

I hereby certify that My Full Name, son of father's name(Father) and mother's name (Mother), was born on date at time in city, state, country.

Signature of Father and mother

I hereby affirm and declare that the above information is true and accurate to the best of my knowledge

Name and Signature by mother

Notary stamps

Question 1: Would they accept the black and white birth certificate (Copy)

Question 2: If not, would they accept both these documents if I provide a letter stating that I have lost the original ?

accept, for what purpose? Birth certificates are no longer required for adult applicants.
 
as stated, not required (unless you have children and you need to prove their relationship to you).

Oh really ! That's a good news. But still wondering, shouldn't I have to show my date of birth somewhere "officially" ?
 
Passport is a requirement.
