Birth certificate affidavit

My spouse does not have birth certificate. I am planning on providing affidavit for the same. Can some one advice me from where the affidavit should be ovtained? a lawyer/attorney from my country or Canada... I am currently staying in Canada.

Also can some one please provide me a sample of affidavit?

Here is the format. Hope this helps..
