Bardcode imm5669

Hello everyone....
I have two questions please..!
1. I heared that there is a recent form ( eimm5669 6. 2019 ) .. is this the one should be filled ??
2. After i filled the imm5669 and after validation . There is no barcode ....
Iam using the the new version of adobe ...
Why there are no barcodes????
