Barcode and cover page

Hello. I have read in several topics something called a cover page what is that?
In addition Regarding the barcode? What do i do? Each one has a barcode do i put the barcode page infrom ofbthe batch of forms for each of us or do i put a copy of the barcodes on each and every form?

You can include the barcode in the beginning of each forms. it should be alright.
