Background declaration-FORM

Hello, just needing some help with completing the new Background declaration form. There is only room for 5 spaces and they need up to 10 years. What can I use to add the rest of the information? Another piece of paper attached? An addendum? the old form we could just use the + or - sign but these have been removed since October 2018.

Just print same page and continue the rest of the information and previous page write down see attached sheet that’s it
