back ground information transcript

In background information needs the name of institution.

1)Do i have to fill the colleges name or university name?
university name printed on the degree certificates
both colleges university name mentioned in the transcript.

2)we have two kinds of transcript.. academic transcript and syllabus transcript.
do they need both or just the academic transcript..
