If they applied together and medical is required for people in Manila (as obvious from the spouse getting medical request), the child should also have received medical request.
If it has been a day or so, may be you get it a little bit latter. If not you should check with the embassy.
The only differences are that there are no x rays and blood tests for kids below 12, no urine test for those below 5. So only physical examination and history if any is asked for kids.

We applied together and when medical requests were received, those didn't include my elder son aged 7. I emailed CIC and received medical request for him 4 days latter. Similarly for another friend who were a family of 3 (himself, spouse and child), the spouse's medical came about 2 days late and they also had emailed CIC asking about it.
