5. Select 2 pieces of identification (ID) that you'll provide. If applying for your child(ren), provide their IDs (not yours). (required)
Both IDs must show the applicant's name and date of birth. 1 must have a photo on it.
Option 1: Passport or official travel document
Option 2: Driver's license (if applying for a child under 18, upload their driver's license)
Option 3: Canadian health card
Option 4: Government-issued ID (for example: hospital card, school records, or health insurance card)
Option 5: Other government-issued ID. Select if applying for a child with only 1 ID. Upload a letter explaining why the child only has 1 ID in the next step.
My son was born in Africa 2 months ago. The only document i have is a birth certificate from Uganda. On the application under personal identification i have to select two options despite not having a passport or government id for my son.
