Asivad and Experts - got request for additional doc please help

Hi Asivad and Experts,

I got request letter just now in my CIC account. Request letter says in order to proceed with the application we need following documents

1 UCI Number Then my name : What must i provide here
2. Schedule 4a
3. Schedule 4b.

Where must i get these documents. Looks like schedule 4a is business nominee form. Do i need to fill this as N/A or just ignore the request.

Please provide me the link for these documents.

1. Provide your UCI number. If you don't know what that is, you probably answered 'Yes' to some question somewhere about having applied to CIC earlier. Tell them now that you don't have a UCI because you have never applied to CIC for anything other than EE.

2. Submit a completed Schedule 4 form.

3. Upload a short note explaining that Schedule 4a doesn't apply in your case.

4. Schedule 4b?

Sorry it is Schedule 4 and Schedule 4a.

I got the forms. Below are my question

Schedule 4 : Do i need to fill it online. or print out fill it manually and sign and den scan again.
Also what should i mention in Funds. Same amount what i mention in EE profile. Also wat should come in Provincial stream/Category. Is it human capital priority stream.

Schedule 4a : Can i upload just LOE that I am not elegibe for 4a and ignore the form. And can i use that LOE to upload in placeholder of Schedule 4a form.

What I am saying is in a request letter at top left they have mentioned in UCI number.
Then body of the letter says you need to provide us the following documents :
1 again my UCI number is mentioned and den my name : what must i provide here or its just typo error in letter and only 2 documents needs to be provided
2. Schedule 4a
3.Schedule 4
