Applying for an OPEN WORK PERMIT along with my WORK PERMIT

Hello all,

I am an Indian Citizen, currently working in USA. I have few questions on Spouse's Open Work Permit (OWP).

a. I would like to apply for Open Work Permit for my Spouse along with my work permit application ? Please let me know if I can do that?


b. If it so, what additional forms to be filled?
