Applying for a visit visa

hi,
i want to apply for a visit visa of Canada with my family from Pakistan. can anyone please guide me what documents are required and what to do to avoid any refusal.
we travelled to UK, UAE, Singapore, Thailand, Malaysia, Qatar, Oman
and have valid 5 years USA Visa.
what are the chances ?
