Applying for a new RAMQ (Quebec Health Card) for spouse who is on WP while PR application is pending

My spouse applied for PR (Quebec). Been 11 months and we are still waiting for the approval.
Spouse has received Work Permit recently. Spouse is in Quebec currently.
I am not clear which options to choose.
