Apply gcms note

Hey all
Before I applied only sponsor GCMS Note but now I want to apply applicant and sponsor both GCMS notes so I want to confirm that sponsor person have to apply both separately like she have to apply for applicant first and then her self like she have to pay 5 dollar for each right?
And if she Appling for applicant she need content form from applicant with sign right?.

Its just 5.00 only not 5 for each.. and yes the applicant need to fill out the consent form

The sponsor can apply and get both sets of notes. You will need the consent form signed by the principal applicant.

OK so in consent form section Ai should mention Application name and in section B my sponsor wife name and in section C my spouse sponsor name right?
