Application to Change Conditions, Extend my Stay or Remain in Canada as a Worker (IMM 5710)

Hi,
I have a doubt related to that. I am close to finishing my IEC WHV (open work permit), and I will apply for a new open work permit through my girlfriend's sponsor. The form IMM 5710 Application to Change Conditions, Extend my Stay or Remain in Canada as a Worker, says that in question number 3:

Check the appropriate box or boxes to indicate if you are applying for:

• A work permit with the same employer
• An initial work permit or a work permit with a new employer
• Restoration of your status as a worker
• An initial or an extension of a temporary resident permit

I don´t know what I should exactly mark. I doubt if it will be only the second one or the third one.
