application returned due to unauthorised representative plz reply urgently

stupid as it may sound but we have been scammed by our representative hence our application is returned. we didnt get to know this until our application was returned by CIC. CIC has notified us that we may have been using unauthorised representative and have asked us to review our application or we can get banned to apply for immigration for two years. our representative did not include the "use of representative form" and simply attached a letter that they are applying on our behalf.

Now we want to resubmit our application by ourselves so what would be the procedure to resubmit under these circumstances.

either we resubmit the application without the "use of representative form" or

submit the application with "use of representative" form to cancel the representative with a note explaining our situation

or resubmit with the objection letter and a not explaining our situation?

you can resubmit your application by filling and signing it again without a representative form, plus excluding any information in all the forms related to your previous representative if any (e.g phone, address, email etc).
carefully go through all the information in the forms again and make sure to re-submit the updated forms(most of the forms have date printed at the bottom e.g IMM 0008(06-2014) make sure it matches the updates forms which CIC is accepting now.
