Application Questions

Hi, I have nearly completed the application but I have some KEY questions that I need answering please.

Question 1

ADDITIONAL FAMILY INFORMATION (APP FORM IMM5406) http://www.cic.gc.ca/english/pdf/kits/forms/IMM5406E.PDF

So far, for this application form I have completed two separate individualised forms, one with my details (principal applicant) and with my spouse's.

However, I'm not sure if I also need separate individual copies of NON-ACCOMPANYING individuals such as my siblings and half-brothers and sisters?

Also this app form asks for the address and details of half sisters and brothers, however I have limited contact with them and don't know their full address or details. In this instance should I explain in more detail on a separate piece of paper and write address "NOT KNOWN" in the application form?

Question 2

Application Guide Part 3: Country Specific Instructions http://www.cic.gc.ca/english/pdf/kits/guides/3901e.pdf

This sates the following:


"...Provide national identity cards, birth certificates, baptismal certificates or other official document confirming relationship for you and each of your family members..."


Does this mean that I need to provide birth certificates of myself, children and also NON-ACCOMPANYING individuals?

And as for Identity cards none of my family members ACCOMPANYING or NON-ACCOMPANYING have these ID cards...Should I state this in a covering letter?

Question 3

I have two NON-ACCOMPANYING children that I don't live with do I need to use this form (I have already included their details in application form IMM0008 - Generic Application form fo Canada).

GUIDE 3999 - says that I (the principal applicant should fill out the form on behalf of each of my dependants however...GUIDE 3901E says that I should fill this form if there are more than 5 family family members. So I'm not sure whether I should fill this one out or not see as I have two kids...?

1. Additional family information: For you and your spouse there are already separate forms. It is OK.
No need to separate the non accompanying members in another form.

If their full address is not known give their city, province/state and country. If that too are not known, you are helpless. Proceed as you said.

2. These requirements are only applicable for the accompanying family members. For you and your children. If you or children have no other identity cards other than the passports, you can write None and not applicable as the case may be. If you wish to add notes about it on the covering letter, it is fine.


3.If your 2 kids are accompanying or not accompanying you have to fill that columns.

You said you have already done it. Then you don't have to make another additional dependent form again.
