Single mother of a 8 year old boy who lives with his Dad. She is applying by herself - does she need to include any information about her son? If, yes, does she need to attach any document such as school records for her Son?

No, it is not necessary for her to apply for her child. No documents are required, just complete the section on children with the information she has, include a note that the father has custody.
