You just have to show that you really tried really hard to get the psychiatric assessment required for your application. For example, don't say "you heard getting the report could be 6 months". Go find out for sure. Start calling the clinics, etc. in your area and ask. If after all the efforts done, you can't produce the report in 90 days, then you can send a letter in to your visa officer and probably attach some kind of information (such as appointment date, etc.) showing that you really cannot get it done by 90 days and you have the appointment made with the psychiatrist.
Most likely when you show your visa officer you are doing your best, they most probably will grant you an extension. But be sure to let them know ASAP when you find out.
