Hi All,

Do any one know DMP other than Dr Chmiel Boguslaw in Chicago.? Appreciate your help. Iam in Wisconsin. And WI don't have any DMP, so i need to see a DMP in IL, which is nearer to me.

Also the DMP link is not working. Any idea where to look for DMP.?

Today i got my medical request. I live in Wisconsin and as there no DMP in WI, i need to go for a DMP in Chicago, ILLINOIS.
From seniors, i saw we should avoid Dr Chmiel Boguslaw. Please help me if any one knows good DMP in Chicago Area.
