Affidative?

Hi,
I am translating and certifying some documents. In the certified copy, the translator of the notary signed that he did the translaion. Do I need to provide the affidative as well? In the notary of our country, I was said that translator*s name and signature under it should be enough!?

Must prepare an Affidavit of a translator and enclose it. Otherwise they will refuse the application .

In my document, it says that the document is translated by translator x, then he signed. Bellow, the notary states that this translation is done by a tranlator he trusts (translator of the notary).
Do you think it is enough?
