Affadavit for Birth Certificate - Template (URGENT)

Hi guys,

I am applying for my PR under the CEC category next month. My birth certificate does not contain my name and has my parents' names misspelled. As per my post earlier and other threads that I read my solution was to submit an affidavit. I am getting it drafted myself and will get it signed by a Commissioner of oaths. My questions are:

1) Is it advisable to draft the affidavit yourself? Or is it better going to a lawyer and getting it drafted?
2) I do have my birth certificate but with name missing (it says baby boy) and my parents' names are spelled incorrectly. Do I submit a notarized copy of that along with the affidavit?
3) If anyone has done something similar before, can you please take a look at my affidavit draft and tell me what you think?
