Should aadditional sheets that we need to attach for sponsorship forms, need to be signedand dated by the applicant or the sponsor??

It is not a requirement.

Here is what needs to be on additional sheets (from the guide):

If you need more space for any section, print out an additional page containing the appropriate section, complete it and submit it along with your application.
