additional family information form

Hello Everyone,

In the additional family information form, it says "Complete all names in English and in your native language (for example Arabic, Chinese, Japanese etc. etc. etc..)"

what do they mean by saying complete the form in english and in native language?? are they asking us to submit 2 copies of this form one in english and another one in one's own native language?? it seems confusing ???

a similar question appears in Schedule A – Background/Declaration (IMM 5669). "Indicate your name in your native language or script (if applicable).

For example: Arabic, Cyrillic, Chinese, Farsi, Korean, Japanese characters or Chinese commercial/telegraphic code.


so what should be my written here??

thanks for ur rply..so u mean to say that in the family info. form, I have to first write the names in english in those boxes, put a slash like this (/) and then write the names in my native language??

please correct me if i m wrong?? but this form filing in native language didn't make sense to me.. ??? ..why they need the names only in native language??
