Additional Family Information- Form 5406E

Help!

I have been browsing old threads for an hour and can't find a clear answer.

I am American (wife) being sponsored to Canada by my husband. We have NO children, I have NO siblings. I am the ONLY ONE coming to Canada

For the Additional Family Infomation:

-Do I list myself as the "Applicant" and my husband as "Spouse"?
-Do I list MY parents or my husband's?
-Do my parents fill out their own form? (they're not coming to Canada)
-Does my husband complete a form too?

Yes you are the "applicant" and your husband as the "spouse"

List your parents name

No.. your parents doesn't need to fill up the forms

There are 2 applications right? one for your husband and one is yours...check the document checklist on his part of the application to see if he needs to fill up the form...

Just to be clear, the sponsor does not complete IMM5406.
