Additional Family Info IMM 5406 - How to Fill?

Dear Friends! Need urgent help!

Our timeline is as follows:

AOR 16 Dec
MEP/IP1 28Dec
NA2 03rd Jan
ADR/IP2 24th Jan

Today we received Additional Document Email requesting:
(a) Separate signed IMM 5406 form for me and my wife
(b) Details of Military Service for me.

Question : Will my name go in the "applicant" section or the "Spouse" section in my IMM 5406E,? (My wife is the Principal applicant)
